Supervisor Approval of Civic Engagement Hours

Welcome! Please use the form embedded below to confirm and approve civic engagement hours for a Renée Crown Honors student.

You should have received a notification email with a unique Civic Record Number. When you fill out the approval  form below, click on the '+ add' button and enter the Civic Record Number you received in the email.

If you did not receive an email or do not know the number, please email Niki Swackhamer (dmswackh@syr.edu) with your name, the student's name, and your organization. In the email, please indicate your approval of the student's participation with your organization.
